2.Application guide

If you wish to apply, please prepare for the application by checking the following documents.

Also please obtain a copy of the latest Application Guide from the Graduate Student Affairs Section. It is available at our window. Or you can obtain it via postal mail too. For details, please see "5.Obtaining a copy of the Application Guide".

If you have a difficulty in submitting the application documents by post, you may submit them online. In that case, please follow the instructions in the Application Guide 6. Application(3).

Other important information

Please tell the staffs which Application Guide you require, i.e. Master's Program, Professional Degree Program, Doctoral Program, or Doctoral Program in Medicine and Desired Department.

Requesting a copy of the Application Guide by post

Mailing of the Application Guide may be delayed if you fail to indicate the program for which you wish to sit the entrance exam (particularly for the Doctoral Program and the Doctoral Program in Medicine) and your desired department. Therefore, please confirm your desired program and major before lodging your request.

Request form etc.

The Application Guide can only be obtained either directly from the Graduate Student Affairs Section counter or by postal mail (phone or fax orders will not be accepted). To request a copy of the Application Guide by post, fill out request form (1) below and send it to the below-mentioned address together with a self-addressed envelope (2). Be sure to write '大学院募集要項請求' ('Application Guide Request') in red ink on the front of the envelope.

1. Application Guide Request Form (download form)
   If you cannot download the form, write down the type of Application Guide you require along with the name of your desired program and your contact details on a piece of paper and send it together with the self-addressed envelope.
2. Self-addressed return envelope
   Fill in your name and address, and attach the required postage to the envelope.

If you require express delivery, add the express delivery fee to the postage fee listed on the left. Also, please write '速達' on the reply envelope with red ink.

If your sending address is outside Japan, please enclose sufficient IRC(International Reply Coupons, 1 piece of IRC=130 yen)with the request form. We will send application form by EMS(Express Mail Service). If IRC is not available in your region, please enclose International Postal Money Order.

Our weight of the Application Guide is about 200grams. The cost depends on the area where you receive. Please confirm here(the rate from Japan post) before sending it.

When you enclose IRC or International Postal Money Order, please also include the EMS dedicated envelope or an envelope larger than 240×332mm, 9.45×13.07 inch. In this case, you do not need to fill in your name and address on the envelope, but please make sure to write your address on the "Application Guide Request" form.

Other important information

You must submit your university/postgraduate transcripts etc. when filing your application so be sure to request the Application Guide well in advance of the application deadline to allow sufficient time to collect all necessary documents.
